Can you track and or reverse an unknown, Unauthorized transaction that occurred. All of my ADA was wiped out. I have the transaction information available. How can something like this happen?

Nope thats not possible. You probably leaked your seedphrase somewhere.

Iā€™m so sorry to hear that. But any transaction that has been made could not be reversed. I think somebody hacked either your computer/mobile phone. Or your seed phrase (15-words/24-words) has been leaked like @Zyroxa said above.

Either there is a malware in your PC/mobile phone or you put your seed phrase digitally online.

You can increase the protection by using hardware wallet such as Ledger + combined with Daedalus/Yoroi.
